Change of Address

As soon as you have your new address:

  • Active Employees
    • Log-on to Workday using your CNet ID and password to update your address.
    • If you move to an area where your HMO does not have providers, you may need to change your medical plan within 31 days of your move.
    • To change your Primary Care Provider, contact your medical plan.
    • Visit HealthEquity (WageWorks) to change your elections for the Qualified Transportation Program.
  • Terminated or Retired Employees
    • To change your address with a University of Chicago Health Plan, contact Benefits at Benefits@uchicago.edu.
    • Contact TIAA at 800.842.2776 to change your address on your retirement plans.
