Active Military Service Leave
Employees in the Reserves or National Guard who are called to active duty will be granted a leave of absence, along with the opportunity to continue certain benefits for a period of up to 24 months.
How to Apply
Submit a completed Personal Leave of Absence form to your supervisor as soon as you receive your deployment notice for active duty. You must provide a copy of your orders for deployment.
Your supervisor will send the completed leave request form and a copy of your deployment orders to the Benefits Analyst - Leave Programs who will complete the final approval.
Be sure to indicate the benefits you want continued during active duty leave. You will be billed for your portion of the premiums for up to 24 months or as required by Uniformed Services Employment and Reemployment Rights Act (USERRA).
Upon completion of a leave for military service, you will be eligible for reemployment. Notify the Benefits Analyst - Leave Programs upon your return from leave.
Please refer to Personnel Policy 517 Leave of Absence for Active Military Service for eligibility, usage, and return to work guidelines. Employees represented by a union are governed by the applicable collective bargaining agreement.
